چکیده

Introduction Activation mapping of ventricular tachycardia (VT) is rarely accomplished owing to limited temporal and spatial resolution, unacceptably long mapping time, and hemodynamic instability. Entrainment mapping is a reasonable approach to identify targets for ablation in patients with tolerated postinfarction reentrant VTs; however, it often does not allow delineation of the entire VT circuit. Introduction of newer mapping technologies capable of rapid and highresolution electroanatomic mapping may allow detailed activation mapping of macroreentrant VTs, enhancing our understanding of macroreentrant circuit geometry and electrophysiology to facilitate ablation.

Endoaneurysmorrhaphy and cryoablation for postinfarction left ventricular aneurysm with ventricular tachycardia.

BACKGROUND Early reperfusion in the acute phase of myocardial infarction and better medical treatment of consequent heart failure and tachyarrhythmia have decreased the incidence of massive myocardial infarction, left ventricular (LV) aneurysm and also postinfarction-sustained ventricular tachycardia (VT). However, for a number of patients, surgical ablation combined with aneurysm resection and...

An underrecognized subepicardial reentrant ventricular tachycardia attributable to left ventricular aneurysm in patients with normal coronary arteriograms.

BACKGROUND In patients with apparently normal hearts, ventricular tachycardia (VT) may only involve the subepicardial myocardium. METHODS AND RESULTS Four patients with exercise-induced fast VT with right bundle branch block morphology were investigated. ECG showed a small q wave in leads II, III, and aVF during sinus rhythm (SR) in all 4 patients. Left ventricular angiography showed small in...

The Dor procedure for left ventricular reconstruction. Ten-year clinical experience.

OBJECTIVE Surgical ventricular restoration by means of the Dor procedure is a surgical option in patients with coronary artery disease, postinfarction left ventricular aneurysm or ischemic dilated cardiomyopathy with or without ventricular tachycardia. The aim of this study was to evaluate the 10-year clinical experience of this procedure in our institution. METHODS From May 1994 to June 2004...

Surgical Cryoablation of Drug Resistant Ventricular Tachycardia and Aneurysmectomy of Postinfarction Left Ventricular Aneurysm

Heart failure is usually associated with left ventricle remodelling, wall thickening, and worsening of the systolic function. Ventricular tachycardia is a common and a negative prognostic factor in patients with endocardial scarring following myocardial infarction and aneurysm formation. The authors present a case of a 51-year-old man with ischemic heart disease, who suffered myocardial infarct...

Surgical ventricular restoration with CABG with or without mitral repair: is it an ideal surgery?

Methods From April 2005 to April 2013, surgical ventricular restoration was performed in 48 patients (36 males), mean age 57 (41-74) years. All patients presented with angina and/or heart failure and/or ventricular tachycardia. Postinfarction left ventricular aneurysm was present in all 48 patients and ischemic dilated cardiomyopathy with a large akinetic left ventricle in 8. The preoperative l...
